Further information on descriptions Description: Aims:
To provide underpinning knowledge on children and transitions. It will focus on the impact transitions have on children and their families, including transitions from nursery to school as well as life transitions such as divorce of parents, adoption death and illness. It will identify good practice for practitioners in managing these transitions.

Learning Outcomes:
On completion of the unit the student should be able to:
* Identify and discuss transitions that may affect children and their families. These should include transfers from and to settings as well as life transitions.
* Identify and illustrate the benefits and challenges of transitions on children's development and wellbeing.
* Identify and discuss best practice in managing transitions, showing effective communication skills and listening to children and parents/carers.
* Identify and discuss the importance of multi-agency and integrated working when managing transitions.

Content:
This unit will examine a range of transitions that children may encounter such as transfers between services, for example, to child-minders, nurseries, to schools and between schools, as well as life transitions such as illness in the family, bereavement, divorce, birth of a sibling, adoption etc.
The unit will consider the skills practitioners need to demonstrate in order to manage transitions well such as the ability to listen to children and empowering children to make their own decisions, to recognise changes in behaviour and attitudes as well as using effective communication to all persons involved in the transition process.
It will explore how children react to change and identify the benefits and challenges presented by transitions.
It will discuss the importance of multi-agency and integrated working, making appropriate links with practitioners, sharing information and understanding organisational procedures and legal frameworks.
