| Description:
| Aims: To update knowledge on how the changes and challenges associated with the ageing process affect medicines use. To increase underpinning knowledge on the therapeutic management of chronic conditions commonly experienced by the older person enabling pharmacists to support patients through medicines optimisation. Learning Outcomes: On successful completion of this unit students will be able to: 1. Explain the physiological, pharmacokinetic and pharmacodynamic changes associated with the ageing process and how this affects medicines handling. (Medicines Optimisation Tool [MOT]) 2. Explain the impact physical, behavioural and socio-economic difficulties can have on an older person's ability to receive maximum benefit from their prescribed medicines. (Case Based Discussion [CBD], MOT, portfolio task) 3. Critically evaluate the evidence base for the management of key chronic conditions commonly associated with the older person; specifically osteoarthritis, osteoporosis, dementia, overactive bladder, Parkinson's disease and end of life care. (MOT) 4. Explain the pathophysiology, symptoms and patient monitoring factors in the above conditions. (MOT, 5. Demonstrate good communication and patient centred consultation skills to enhance the older person's experience. (CBD) 6. Explain the benefits and challenges of integrating medicines optimisation into multidisciplinary care pathways of the older person and identify the healthcare professionals involved in care of older people across sectors within their local area.
