Making an appointment
To make an appointment with the Student Health and Well-Being team you will need to complete the registration form below. You will need to confirm that you have read and understood and agree to the Student Health and Well-Being Code of Professional Practice and Confidentiality and Data Protection statement/Terms and Conditions by ticking the consent box before submitting your form.
It is important that you provide as much information as possible to help us allocate you for most appropriate support to meet your need, this may be face to face individual counselling, individual CBT, on-line counselling(email), joining a group or workshop or meeting with a Mental Health Practitioner. You will be offered an assessment session with an appropriate team member. We try to offer further appointments with the person you first saw but in some cases referral onto another team member, into a group or external agency may be more appropriate to meet your needs.

IMPORTANT - YOUR APPOINTMENT OFFER
- Application forms submitted after 4pm, during the weekend or on Bank Holidays when the Service is closed will be looked at the next working day.
- You will receive your appointment through SAMIS/ESD and will receive an email with a reply link. This email will be sent to your university email address. If you have not heard from us within 1 week please email listening@bath.ac.uk
- It is important that you respond to either confirm your appointment or request an alternative date (this may entail a longer wait). If we do not hear from you this may result in the appointment time being offered to another student.
- If you confirm your appointment and do not attend you will need to contact the service to request a new appointment. You can do this by emailing listening@bath.ac.uk and you will then be added back on to the waiting list. If we do not hear from you your enquiry will be closed.*
*Each case is reviewed before closing and you may be contacted by a member of the team for further information.
