Drug use and safety in pregnancy
The pharmacoepidemiology group has an interest in investigating the utilisation and safety of prescribed drugs in pregnancy. This work is being undertaken using the General Practice Research Database (GPRD) where all pregnancies and outcomes have been identified resulting in over 1.2 million pregnancies available for study. Present work includes a project to investigate the utilisation of asthma medications in pregnancy. Likely exposure before and during pregnancy will be determined and mapped by trimester.
Drug use and safety in older people
Older people have the greatest use of prescription medication for the prevention and control of chronic diseases. Prescribing in older people is complex as pharmacodynamic and pharmacokinetic properties change with age: the time taken to clear drugs from the liver and kidney increases as the functionality of these organs decreases with increasing age. Studies have been undertaken to determine the utilisation and safety of specific classes of drugs in older people, for example antipsychotics and antidepressants, however it is important to consider the wider picture: older people are regularly prescribed more than one drug and often have comorbidities that need to be taken into account. Older people rarely take part in clinical trials of medications therefore there is little information available on the safety of medications particularly in the oldest old or the very frail populations or in those with many comorbidities. Key areas for investigation include the utilisation and safety of drugs used long term, in particular the combinations of drugs prescribed, likely indications, comorbidities, key cascade prescribing patterns and how prescribing in older people differs with age. Specific disease areas of interest include disorders of the central nervous system such as Parkinson’s disease, schizophrenia, dementia and Alzheimer’s disease. In studying these diseases in older people consideration must be given to other chronic conditions that they may be suffering from including heart disease, rheumatoid disorders and diabetes; we have extensive experience in studying these disorders using the GPRD.
