Most of our research projects involve use of healthcare databases. Between us, we have over 20 years experience of working with the General Practice Research Database (GPRD), a database renowned world-wide for its quality and use for epidemiology research. In addition to the GPRD, I have worked with databases in the Netherlands, Denmark, and Scotland. Our research focus is as follows:
Drug Utilisation and Safety in Vulnerable Groups (such as pregnant women and children)
We have identified all pregnancies recorded on a combination of two versions of the GPRD, namely the version available from EPIC and the version available from the GPRD at the MHRA. Combining these two data sets results in about 30% more data than is available from one source alone; this is important when investigating adverse events in pregnancy associated with drug exposures since statistical power becomes an issue.
As a result, we have information on about 715,000 pregnancies beyond 28 weeks of gestation and about 290,000 spontaneous abortions and terminations of pregnancy. For the data up to April 2007, mother-baby pairs have been identified successfully for about 80% of pregnancies. We have carried out a preliminary study of folic acid antagonists and orofacial cleft, a study investigating the utility of the GPRD compared with a pregnancy registry, and a preliminary study investigating the risk of stillbirths in women with diabetes comparing stillbirth risk associated with different diabetes treatments. In these studies we were able to identify known risk factors and it was encouraging that frequency of recording of stillbirths, as well as the frequency of orofacial cleft was comparable with that found in other sources. Following on from this we are conducting further validation work as well as safety evaluations.
Risk Management and Pharmacoepidemiology using Health Care Databases
Our experience using health care databases in the area of pharmacoepidemiology has stimulated our interest in methodological issues associated with using health care databases for purposes of safety evaluation. As a result, we have conducted research with these databases in a variety of disease areas, especially diabetes mellitus, cardiovascular disease, prostate cancer and rheumatoid disease. A lot of our work has involved hormonal products both for prostate cancer and in oral contraceptives and hormone replacement therapy (please see list of publications below).
