Academic Probation Review Form
Use this form to report your progress against your probationary objectives to Academic Staff Committee at your next review as a probationary lecturer.

You only need to complete one form dependent on your probation type:
Lecturers (teaching ) should complete Form 1
Lecturers (teaching & research) including Pre-probationary Lecturers, should complete Form 2
Once completed by yourself, HoD/Reviewer and Dean these forms must be submitted to the Academic Career Progression Team.
