Academic probation review form

Use this form to report your progress against your probationary objectives to Academic Staff Committee at your next review as a probationary lecturer.

Detailed information about how to complete a probation review form can be found on our academic probation web page and in the guidance within the form.

You only need to complete one form dependent on your probation type:

  • Lecturers (teaching) should complete form one

  • Lecturers (teaching & research) − including pre-probationary lecturers − should complete form two

Once you, your HoD/Reviewer and Dean have completed these forms, they must be submitted to the Academic Career Progression Team.
