Designated Maintainer authorisation form
Designated Maintainers can use Access Manager to maintain people's records. Use this form to gain authorisation to become one.
The form should be returned to the Computing Services Security & Identity Management Team, 4 East South, either in the internal mail or by scanning and emailing to email@example.com. To remove Designated Maintainers, email your request to firstname.lastname@example.org.
