Student complaints form
If you intend making a formal complaint, you should use this form to put forward your case.
If you intend making a formal complaint about the delivery and quality of services received, or about the delivery of teaching/supervisory provision or any other matters relating to a programme of study, you should use this form to put forward your case. Please ensure that you have read the University’s Student Complaints Procedure and that you have attempted to resolve the complaint informally before submitting this form.
This form should be sent to the Head of Department/Service which provides the service to which the complaint relates, and copied to the Office of the University Secretary.
