Food supplier questionnaire

A questionnaire for all prospective food providers working at the University to demonstrate that they have the required food hygiene systems in place.

All food providers and food businesses supplying food for University managed events − where the caterer is under the direct supervision of the University − are required to complete a food supplier questionnaire. This includes off-campus events where the University is engaging a food supplier to provide catering at a third party venue.

If the venue has its own in-house caterer then this check is not required. The event organiser should check that the venue has a suitable Score on the Doors rating of at least three stars before booking any venue.

This enables the University to check that the suppliers we use have the necessary procedures and arrangements in place to meet food safety and food hygiene requirements.

Completed forms and all supporting evidence should be returned to Cheryl Herbert at cgah20@bath.ac.uk for approval.

Orders should not be placed with any caterers before confirmation has been provided that the caterer has satisfied the checking process.
