Human Tissue Risk Assessment Form

Researchers working with human tissue must complete this form to comply with the Human Tissue Act (2004) and HTA Codes of Practice.

Form

All research projects using Relevant Materials as defined in the Human Tissue Act must submit a risk assessment considering potential areas in which the project could possibly fail to comply with the Human Tissue Authority codes of practice.
