Probation forms

Use these forms to review performance and set objectives during the probation period. Lecturers should use the process and forms for academic probation.

Form

Start of Probation

  • use start-probation-form-ebf for departments who have implemented the effective behaviours framework (EBF)
  • use start-probation-form for all other departments

Mid Probation

  • use mid-probation-form-ebf for departments who have implemented the effective behaviours framework (EBF)
  • use mid-probation-form for all other departments

End of Probation

  • use end-probation-form-ebf for departments who have implemented the effective behaviours framework (EBF)
  • use end-probation-form for all other departments
