Staff at the University of Bath can complete this form to request permission to carry out a survey with over 250 students in two or more departments.

If you a member of staff and are contacting over 250 students to take part in a survey and if your student sample is taken from more than one Department or School, please complete this form and email it to the Student Engagement Team. They will schedule your survey at an appropriate time.
