Information for participating clubs (Men's teams)

Thank you for participating in the Injury Surveillance Project for the 2019-20 season.

  • If you choose to report information using the online form, you will be provided with a link and log in details via email. This email will also contain user instructions.
  • If you choose to use paper forms, you should receive an Information file containing all the injury forms, pre-paid envelopes and supporting information.

This page provides specific details for each section contained within the information file.

Information pack contents

1) Form Guidelines

  • These provide some written guidelines for the Project and help on some of the sections of the Injury and Match Report forms.

2) Player baseline information

  •  This section contains a guide on how to complete this form
  • After completing information for each 1st XV squad player, you should retain the yellow copy and return the top white copy to the study team.

3) Player information and consent

  • Each 1st XV squad player should read this information sheet  and contact the club's injury manager if they wish to withdraw from providing their injury information to the study team.

4) Match Report Forms (new)

  • Sample Match Report Form
  • Duplicated Match Report Forms are supplied for each 1st XV match. 

5) Match Report Forms (complete)

  • Section to store completed (yellow copy) Match Report Forms.

6) Time-loss Injury Forms (new)

  • Sample Time-loss Injury Form
  • Duplicated Time-loss Injury Forms for injuries which keep the player out of match play for more than one week. 

7) Time-loss Injury Forms (complete)

  • Section to store completed (yellow copy) Injury Forms.

8) Player Master List 

  • The 'Time-loss injury master list' is sheet is for you own organisation in terms of logging Time-loss Injury Forms which are in progress and when they have been returned.
  • This form should not be returned to the study team.

9) OSICS 8 full list

  • This section contains a full list of three character codes for diagnosing time-loss injuries.
  • The list is substantial but injuries are grouped into body regions and types to make reference easier. When the correct code has been found, each character should be inserted into separate boxes in Section 3.4 on the Injury Form.

10) Envelopes

  • This section contains the pre-paid and addressed envelopes required over the course of the season.
