Some overlap exists with learning outcomes and content covering the Dementias in PA50200 and Osteoporosis in PA50201. You must not cover these therapeutic areas in this unit if you have already used them in assessments in PA50200/PA50201.
Further information on descriptions Description: Aims:
To update knowledge on how the changes and challenges associated with the ageing process affect medicines use. To increase underpinning knowledge on the therapeutic management of chronic conditions commonly experienced by the older person enabling pharmacists to support patients through medicines optimisation.

Learning Outcomes:
On successful completion of this unit students will be able to apply current and recent research and evidence to:
1. Analyse the way that physiological, pharmacokinetic and pharmacodynamic changes associated with the ageing process affects medicines handling in the older person. (Medicines Optimisation Tool [MOT] Case Based Discussion [CBD])
2. Explain the impact physical, behavioural and socio-economic difficulties can have on an older person's ability to receive maximum benefit from their prescribed medicines. (Case Based Discussion [CBD], MOT, portfolio task)
3. Critically evaluate the evidence base for the management of key chronic conditions commonly associated with the older person. (MOT and CBD)
4. Reflect on your own practice in applying the concepts of pathophysiology, symptoms and patient monitoring factors in the management of these conditions in practice. (MOT, CBD, CARE)
5. Demonstrate good communication and patient centred consultation skills to enhance the older person's experience. (CBD)
6. Explain the benefits and challenges of integrating medicines optimisation into multidisciplinary care pathways of the older person and identify the healthcare professionals involved in care of older people across sectors within their local area. (Portfolio task)
