Learning Outcomes:
* Critically discuss the pharmacist's role in managing long term conditions.  
* Critically evaluate the evidence base and apply clinical reasoning and professional judgement to the evidence base for the management of key long term conditions (LTCs), specifically asthma, chronic obstructive pulmonary disease (COPD), diabetes and hypothyroidism. 
* Analyse and critically reflect on your role in reviewing medication and monitoring their safety and effectiveness in patients with the above conditions. 
* Explain the pathophysiology, symptoms and patient monitoring factors in the above conditions. 
* Apply good communication and patient centred consultation skills to enhance the patient experience and promote shared decision-making.  
* Apply commonly used clinical skills relevant to the above conditions e.g. spirometry, peak flow, HbA1c and diabetic foot examination. 
* Safely review medication and optimise medicines use in patients with the above conditions. 

Aims: To update underpinning knowledge of a range of long-term conditions and therapeutics enabling pharmacists to support patients in the self-management of their conditions, through optimising medicines use.
