Individual Mitigating Circumstances report form
Use this form to tell us about your circumstances and the impact they have had on your recent assessments.
Before filling out this IMC form, read our guide to Reporting Individual Mitigating Circumstances.
Submit the form and any evidence you are providing to your Director of Studies up to seven calendar days after the affected assessment. If your circumstances have affected more than one exam, you can summarise each affected exam in one claim, submitted up to seven calendar days after the end of the exam period. Please submit your IMC claim(s) as soon as you can after your affected assessment(s)—don’t wait until the last moment.
