| Aims: | To update underpinning knowledge of key long-term conditions and therapeutics enabling pharmacists to support patients in the self-management of their conditions through optimising medicines use. |
| Learning Outcomes: | * Critically discuss the pharmacist's role in managing long term conditions.� � * Critically evaluate the evidence base and apply clinical reasoning and professional judgement to the evidence base for the management of key cardiovascular long term conditions (LTCs), specifically Coronary heart disease (CHD), acute coronary syndrome (ACS), myocardial infarction (STEMI and NSTEMI), secondary prevention of ischaemic heart disease (IHD), chronic heart failure, lipid modification in primary and secondary cardiovascular disease (CVD risk reduction), peripheral arterial disease (PAD).� * Recognise the� effect long-term conditions can have upon mental health� * Analyse and critically reflect on your role in reviewing medication and monitoring their safety and effectiveness in patients with the above conditions.� * Explain the pathophysiology, symptoms and patient monitoring factors in the above conditions.� * Apply good communication and patient centred consultation skills to enhance the patient experience and promote shared decision-making.� � * Apply relevant clinical tools and interpret investigations to analyse risk and monitor patients with the above conditions.� * Safely review medication and optimise medicines use in patients with the above conditions.� |
