This questionnaire and review form will help to provide the information to make an initial assessment of a request to work overseas by a member of staff.

Please give this questionnaire to your member of staff who is requesting to work overseas. Please complete the relevant sections of this form along with the attached Overseas Working Review Form before submitting confidentially to your HoD and OWG for consideration.
