You must complete this form if you're applying for South West Doctoral Training Partnership (SWDTP) or University Research Studentship Award (URSA) funding.
If you're applying for SWDTP or URSA funding, you must complete this form and submit it as part of your online application to study.
As well as completing and submitting this form, you must also make sure your supervisor completes and submits their form, and that you complete all other requirements.
