This could be a mental health setting, gender setting, or other NHS service where regular meetings with clinicians are planned.
The purpose is to better understand the individual’s autism-related needs in a clinical setting, covering areas such as:
- communication preferences
- routines
- sensory needs
There are two sections to the form. One for autistic people to complete and one for parents/carers of autistic people.
