You will need to save your completed form and any supporting documentation as a pdf. or other image file. One form and two pieces of supporting documentation can be uploaded. An online form can be completd if preferred when applications open.

If you are a first year undergraduate applicant holding an insurance offer or a restarter currently in suspense, please complete the additional requirements form and send to reslifesupport@bath.ac.uk with your supporting documents.

If you are a current resident student and want to remain in University accommodation next year because your condition significantly affects your ability to live in private housing, you must apply for an assessment via the eligible returner process. Please email reslifesupport@bath.ac.uk to request the eligible returner form.
