New first year undergraduate and postgraduate students with a diagnosed medical condition or disability must complete this form as part of their accommodation application.

If you are a first year undergraduate applicant holding an insurance offer or a restarter currently in suspense, please complete the additional requirements form and send to reslife@bath.ac.uk.
